How does a PG4 look like?
Or are these copies made from the glass.
If so,make sure the optics are clean and also the white plate and do a CCD adjust.
Pg 4 has a background tone top and bottom, middle is lighter.
What kinda toner do you use, original or OEM?

Is potential control on or off.
With everything you've replaced I might start suspecting the laser. If you have one on hand try it out.
could you post a scanned image of the problem copy/print
Check to see if toner save in on. Service mode - copier/option/body/cotdpc-d = 0
