Teckat, Screwtape, guys . . . thanks. The resolution to my problem was that the reader controller PCB had some hardware problem that caused the DADF not to be recognized. The required 5V was not being sent through the yellow wire to recognize the DADF.

Will be sending board back for testing and repair.

    Again, thank you.

Good Luck, hope it works out.

As u can see on Canon's old circuit diagrams they were very secretive on showing the color of signal wires, and the explanation in Service Manuals . Think it's a C.I.A thing.
Five volts DC is an essential supply for signals and operations/ many times those wire ties crimp those small wires, and cause many issues. it's like bending or having a crimp in a water hose, the flow is slowed down

Thanks guys for posts. The consistent problem was that the DADF was not being recognized, and the loss of the 5V on the yellow wire (as well as the originating terminal on the reader controller PCB) was consistent. 3 of the prongs are suppose to have 5V.

I am more concerned though about why the board failed in the first place. I understand these boards are not prone to failure.

Unit is working & monitoring now. Thank you.

Chalk it up to "Shit Happens", don't spend too much time worried about the cause. You have other patients in the waiting room. If it should happen again on that box, then you have something to investigate...
