I believe it is the Set User Information setting, but I don't have the privileges to change that. I believe it is on the print server. I should be in IT. They've been trying to figure it out for two years.

WSD, or Web Services on Devices, is a mechanism in Windows Vista/Server 2008 which allows a 'WS discovery' for obtaining information about printers.
This is a way to update the installed printer drivers without user intervention providing the most current status/data about the printers.
Set all WSD Print settings to off
Additional Functions>System Settings>Network Settings>TCP/IP Settings>WSD Print settings>Use WSD to off

**Knowledge is time consuming, exhausting and costly for a trained Tech.**
"When someone sends a print job to the printer it also prints a sheet with their name on, the print job number, the date and the time. It actually sends it to a different tray than the actual print job. It is called a header for some printers"
by your description of going to a diferent tray and what comes onit, it sounds like someone tried to activate a part of the machines reports and now she is delivering a report for every job done ( so it comes out on the reports assigned tray ) if you send an email from the machine do you get something similar? with that extra sheet you have a report certifying that the print was made, i think that by browser you can access the machines reports section and check it
Life is a learning continuum...
Eliminate the machine end of it. Hook up a laptop with a crossover cable and print one of your documents to it. Still the same?...machine setting. No extra page?...application or server setting.
Thanks to everyone for your suggestions. It is called a Separator Page in Windows Server. We went in to the print server advanced settings and under Separator Page, there was not a option to check, but we deleted the path to the separator file, which was pcl.sep. That solved it. The file name may differ.
Thats it!!! Was trying to remember the setting. I knew it was a server setting. I remember now when printing from Linux through PlanetPress I had to enable it so that machine knew job was coming from Linux. Separator page(not printed but issues instruction) was necessary on each job. Glad you resolved it(....I knew it wasn't the box....)
