For me, Number 1 reason for skewed/crooked copies from ADF is: tray guides not aligned to the paper stack.
Number 2 reason: Endusers leaning on, pushing or just smashing continiosly ADF until it looses parallelism with machine.

Sometimes it’s just cheap crappy equipment. A lot of the smaller machines I work on skew when scanning but not copying. I believe that in copy mode the machine processes the image to straighten it out, but for whatever stupid reason, does not use the same processing for scans. Even brand new machines have had this issue. I’ve gone as far as replacing entire DADFs without any improvement. If it’s not a cracked/bent/broken hinge or anything else previously mentioned by other techs... it might just be the way it’s gonna be. Good luck.
it ended up being the way the ADF/Scanbed aligned. the adf was slightly crooked from key-op leaning on machine. i replaced the scan bed and adf unit and works like a charm now
