HP Pro mfp 521dn

Have a HP MFP 521dn that will not duplex. The duplex option is selected but we get two sheets when trying to duplex. And here is the hairy thing, the second sheet is a mirror image (backwards). Did firmware updates and no help. Anyone seen this?????

Can you do a duplex test page; If it works good then I would think it would be a driver

Does that happen for both prints and copies?
If it only happens when the machine prints, I'd be somewhat suspicious of the application the machine is printing from as well as the driver.
