I currently have four of these DFs doing this. All roughly the same age, so I am currently guessing it was a bad batch. One other local dealer has the same problem, according to Ricoh.
Ricoh cannot figure out what is wrong, have currently replaced belt and reverse roller, clutch, motors, board, firmware, sensors, changed buckle to either extremes, done all the TBs, swapped with another machine and it will follow. Had a Ricoh engineer come and look at one with me, he tried everything from the basics up, and still could not understand what was causing it.
It seems to stop at the same location, and it is very intermittent. The only thing that has so far worked (at least, the customer hasn't phoned in again since) is swapping the DF with a newly released machine. In our case, we took a 2003's DF, from a spare rental.
Ricoh released new firmware June2016 for adf3090
01.290:03Symptom corrected:
- JAM01 or JAM16 may occur when scanning thin originals.
- JAM14 may occur as a result of an unwanted delay in feeding out the last sheet of the original.
- Lifting up the ADF during initialization may cause JAM14 in the process following the initialization.
maby this can help .....?
We currently experience the same on two 3003 models with d779/df3090
jam 14 and 16.
Jamming job when scanning/copying, 2-3 sheets goes fine. Larger job >10 results in jams. the jam is not at the last sheet.
we can get rid of the jams if we put pressure down on the DF on the top, left side of the adrf.
adrf,system,enging Fw upgraded, checked TB
Remove burrs and make sure the feeler moves smoothly (see the RD147105 for a procedure).

we have been changing the feed in unit as a complete unit when you get the unit the rollers that hold the feed belt i think are a diffrent colour so we suspect they have been changed for some reason this has cured most of our problems with customers who like to do a lot of scanning. part number D6842100
