MP 2000 output in wrong orientation

Have a customer who says that the machine will at random output the image SEF, machine has 8.5x11 LEF in tray one and 8.5x5.5 LEF in tray two. Haven't observed it myself so I'm not sure what could cause it. Paper size/orientation sensor under glass seem to be working properly. Not the first time they have mentioned this.

Could be user error but I'm not really sure.

    Ideas?

Dirty sensors on the adf feed table? or maybe the sensor board itself on the table could be a little 'gunked" up
    Just a guess...

Rotate sort could be enabled in the driver. Is the image rotated correctly on the SEF output?

It's 106 miles to Chicago. We've got a full tank of gas, half a pack of cigarettes, it's dark and we're wearing sunglasses.

    Hit it.

Could be they are copying an off size document off the glass with incorrect scan settings.

Copy only, this machine does not have the print option installed. I don't remember if it has a fax but they only mentioned it happening while making copies.

I'll check the ADF paper size sensor as well next time I'm there.
