Make sure all the drums are screwed in.
What is the result of SP2-194-007, and SP2-194-010 to 012?
You could have a faulty ITB or ID sensors.
Looking at your sample, why is the paper so wavy, did it exit the machine like that?

I see the paper is wavy. The Fusing Units in the models are quite sensitive. Could be the fusing unit is causing this especially as you have already tried line adjustment and it is not working. I have had countless problems on these units if the paper is damp, especially if the machine is near a window with no air-conditioner and near the sea (on the coast).
Check that the paper is a good make of paper and not recycled .
I found damp paper causes the wavy effect and also colour offset. if you are in a hot clmate i suggest moving the machine to an air conditioned area to test it and see if this helps.
Failing that put another fusing unit in just to check if this solves the wavy issue.
I have had to change Complete Fusing Units here on the Coast to resolve wavy paper, colour offset and Toner rubbing off on envelopes. (Even though I spent a day making adjustments according to Technical Bulletins-nothing helped. Only complete Unit replacement solved all the issues.)
While color registration is done by a mirror adjustment in the laser housing unit, it could be a problem in there...
Are you sure that sure that forced line position is nor ending in an error? Yellow appears far enough off to prevent forced line position from working.
It looks to me like you or someone else at your location did either a memory all clear (SP 5801-001) or engine memory clear (SP 5801-002). If you do not have the birth certificate or a non default print from before the problem started, you may have to do a manual line position adjustment.
If you look under the front cover you may be lucky enough to have the birth certificate in there. Re-input all the values and it should bring line position back into adjustment enough that the machine can adjust it from there.
Oh, and tell the "sales person" he's a fucking idiot.
It's 106 miles to Chicago. We've got a full tank of gas, half a pack of cigarettes, it's dark and we're wearing sunglasses.
Hit it.
Typical Salesperson! Yes man made faults can be the most difficult to find!
I am not sure if Ricoh still keep birth certicate on file at corporate location for 7years. It doesn't hurt to call them up.
