    ok, i assume, that you have the firmware up to the latest and greatest in between

    try setting the country setting to "other", this will spread the tolerance window as wide open, as possible. (dont forget to set the dialing method back to tone)

    I ask again: sure, that your customer has no VOIP ?

    As a last resort, you can try the following, but i doubt, that this is helpful in your case. Anyway, if all else fails, its worth a try:

    enter fax service mode like in the picture:

    change RAM-Adress:
    00680576 to 01
    00680577 to 05
    00680578 to 05
    00680582 to 05

    To cut long story short: you are using your internet connection to transmit your phone calls. (for detailed information ask google and wikipedia)
    The main problem with this connections is here to read:

    Support for sending faxes over VoIP implementations is still limited. The existing voice codecs are not designed for fax transmission; they are designed to digitize an analog representation of a human voice efficiently. However, the inefficiency of digitizing an analog representation (modem signal) of a digital representation (a document image) of analog data (an original document) more than negates any bandwidth advantage of VoIP. In other words, the fax "sounds" simply do not fit in the VoIP channel.
    This leads to effects, you described, and is a pain in the ass to get rid of.

    in fact, you are impelled to screw the fax modem down to 9600 bps, disable ecs and more settings are necessary; for detailed information about ricoh fax modems settings for VOIP contact me after you have made sure, that your customer really uses it

    customer just confirmed that they do not use VOIP. still no luck
