Most common fault is the toner hopper, I'd start there then work toward more complex troubleshooting. Pop the hopper apart and check for the mylar paddles on the inner portion of the hopper, chances are some or all are missing.

No luck with this one. Here's a list of things I've made to it so far:
- Cleaned toner hopper (all mylars are in place)
- Replaced ID sensor with new one
- Updated firmware to latest version
- Swapped developing unit and toner hopper with another working machine
- New drum and blade
- Experimented changing toner supply mode to pixel count
- Swapped toner bottle motor with another working machine
So far, and even though I've been trading parts with another machine sitting side-by-side next to this one, the problem remains. I really have no idea of where to look next. Maybe a sledgehammer?

Change the hopper, Not worth repairing.Pop the hopper apart and check for the mylar paddles on the inner portion of the hopper

Try putting a piece of paper between the toner supply unit and the dv unit blocking the flow of toner to the dv unit. Run some copies see if you can and see if toner is building up in the supply unit. this may help eliminate the toner supply to the dv unit as the problem
It's 106 miles to Chicago. We've got a full tank of gas, half a pack of cigarettes, it's dark and we're wearing sunglasses.
Hit it.
I put a mark on the toner bottle and run copies or use the output check for the toner motor. I have seen where the toner motor will turn without the bottle in there but the load of turning the bottle stops it.

With the toner sign always on, first to check with the print out copy,is it under tone or overtone? if undertone the problem is the toner is not supplying to the tank, if overtone the problem is the td reading is out. From the SP 3-103-1,2,3 to confirm the reading of the td and id. Make sure that you are using OEM toner.
I agree with every one ,but do you have ORIGINAL Ricoh toner.
