Can you post how you resolved this issue please? We have been advised by Ricoh to replace IOB and controller so far with no luck, possibly NVRAM next after seeing your post.
Ricoh seem to think otherwise then! We have an mpc 5054 included in the devices not scanning, all brand new Ricohs anyway i think.
Can you give specifics regarding proper firmware? these devices...

We have a customer who did the same.
I called Ricoh tech support (straight to second line) who advised Ricoh only work on v1.. nothing can be done.

what i do is stretch the sep pad spring to stop it feeding the next page of paper which triggers the sensor just above the feed tyre.
There's always an explanation for these type of things, and its never the copier!

Firstly i would swap the fusers, i know you said its ok to the internal tray - does the machine sit on processing before the machine finally starts up?
next i would swap the finisher over, see if...
easy fix, anything after 02 models has this setting.

Double check network settings on the device.
Check SNMP settings compared to a working device.
Have you tried manually adding the device to the box using IP address?
We have had similar issues and to resolve our trainers have had to edit the print quality options and colour options within the driver.
my guess is sc400 - dirty ID sensors.

We also designed stickers for the machines that explicitly say replace toner when asked or there may be charges.
