most of the times the originals get skewed along the way and end up bumping on one of the spacer nibs on the guides.
Some cut of these spacers, but it's better to look at the source of the skew.

Try replacing the toner sub hopper, might be clogged. (prepare to undo a lot of screws!;))

Did you update the firmware of the machine? especially the java update fixes a lot of issues with PCC.
The interupting will only begin after the job is completely scanned, so when Sorting is enabled and you scan from the exposureglass it will only start after you press '#'
I dont know if that is even an option. When the forward fails the machine gives a 'Network Error' in the fax function, and stores the fax in the memory.

You can easely check by pushing the duplexunit inwards while printing a test page. if it comes out good (or at least better) it's the ptr pressure.
Looks like dirty developer to me too, or dust on the shield glass. You can check by detaching the developer unit from the drum assembly and checking the mag roller.
