Check your Cyan toner hopper, might be packed with hard toner. There is just enough not to trigger low toner, but not enough to do the print job.
Check the grounding strip behind the flywheel. Might be loose or dirty.
Check the Toner Hopper. The transport auger could be broke and the tube is clogged with toner. I have that happen to two machines of that model.
Kind of looks like one of the mirrors is misaligned or tweaked. Maybe scanning motor tension spring broke or popped loose. Could also be Focus lens going bad.
Do your scans/copies look the same if you use the ADF or using the glass?

Check the tension spring that holds the toner bottle in place. I have had those pop lose a few times on those models.

Is it from one or both trays? I had something similar happen to me from Trey 1. Never found the problem, so ended up replacing Trey 1 feed unit and that fixed it.
If all else fails, take the shutter off. It will work without it, will just have to clean ID sensors more often. Of course, it all depends on usage.
Looks like you need to clean/flush the print heads. I know that if cleaning is not done daily, the print heads will start to gum up.
