You must check the waste toner main drive which is mounted in the back of the machine.
That will be your culprit.
My bad, that transport assembly kind of sticks out of the machine when the two halves are separated for shipping, could it possibly have been knocked during transport, so it isn't in correct...
Maybe a broken wire in that fuser guide assembly, is that sensor in the guide that flips up when clearing a jam? I remember the yellow fuser cover catching the wire loom at the top if not careful...

You mentioned the drive belt was loose, have you been able to tighten the tension for the belt?, maybe its as simple as that.
Are you also using the Tiffen scale sheet that comes with the Fiery?
You can adjust secondary transfer settings for heavy paper in service mode, HV-TR, give that a try.
