Totally agree with Zaxcon. Had this problem twice. Bracket on the shift tray has bent causing the shift tray to vibrate at decending. Check the bracket.

Try following. Push in the right hand duplex cover slightly and hold. Do a duplex copy. If it works the duplex gear is not proper engaging to the clutch. Could be the duplex cover hooks worn out....
