The ones that I keep in stock are a Konica Minolta part number:
    20AK 4612 0 Conveyance Guide Sheet

They're about 12" x 1 1/4" with a self adhesive strip ~1/4" wide. I've cut them up for DF's, finisher guides, copier guides, ... pretty much anywhere a LTR page has to travel. They do not withstand fuser heat, not for lack of trying.

If you'd like a serious answer to your request:
    1) demonstrate that you've read the manual
    2) demonstrate that you made some attempt to fix it.
    3) if you're going to ask about jams include the jam code.
    4) if you're going to ask about an error code include the error code.
    5) You are the person onsite. Only you can make observations.

I have cut a transparency and used double sided tape with success more than once.

I found it in the bulletin for the FS534 finisher regulating plate issue. =^..^=
