if this is similar to a 455, then check the black plastic actuator for excessive wear. they get worn down to a sharp point and need to be replaced.
Ordinarily I won't go guessing about jam codes, but this looks a bit like E130/C130 jams. Look at the sector gear on the back of the tray. Manually drive the lift plate up to confirm that the lift plate is high enough, before the sector gear touches the side rail. If you're still not getting enough lift, even with an uncracked sector gear, and the screw tight, you can add a bit of bend to the lift plate to get some additional height.
What jam codes? =^..^=
If you'd like a serious answer to your request:
1) demonstrate that you've read the manual
2) demonstrate that you made some attempt to fix it.
3) if you're going to ask about jams include the jam code.
4) if you're going to ask about an error code include the error code.
5) You are the person onsite. Only you can make observations.

Thanks for the help guys. I found the cause of jam its the mylar that block the actuator to return to position and cause jam. See my attachement i just cut some part of the mylar.
