""This error happens only in A4. """
are you sure bypass ? if not you try on change paper size in tray.
08-225 select A4
08-226 select A3.
and check.

I am taking about the original size. Not the copy paper size. This Jam happening even while scanning also.
Thanks in advance
try to initiate df:
05-356
the do:
05-367,368
Even the 08-356 didn't help. Still the same.
First clean read sensor it can be dirty, second replace read roller, this is a big roller which moves paper it will solve your E721 jams.
I am having the exact same issue. Can you confirm if the change of Read Roller fixed your issue please?
I have cleaned and tested read sensor and it is fine, but the paper (if A4 orientation) just seems to stop before it hits it. This does not occur with A4-R, which scans perfectly fine. The pickup and separation rollers are also brand new.
I know this is an oldie, but it's a truly odd situation.
This is what I found:
These "T" shaped mylar guides just past the white plate were all worn completely through to the adhesive, leaving jagged edges to snag and exposed adhesive.

Naturally I didn't have replacement mylars, but I cut new ones from what I had in stock. They work quite nicely. =^..^=
If you'd like a serious answer to your request:
1) demonstrate that you've read the manual
2) demonstrate that you made some attempt to fix it.
3) if you're going to ask about jams include the jam code.
4) if you're going to ask about an error code include the error code.
5) You are the person onsite. Only you can make observations.
