I am trying to put together a comprehensive site survey to get sales and service on the same page. I don't want to reinvent the wheel here so I was looking for a few examples if some of you are willing to share. Thanks in advance Bob.

Is this in regards to pre-qualifying a customer's needs prior to a sale???

Yes, We are looking at putting a packet together that would have sales rep info to be collected then a service rep would visit to collect there part. Info to be collected would be power and space requirements, stairs / elevator, IT info, environmental considerations ...
