1. Setup Photoshop document with custom size, Say 13.625" x 10.625" @ 72 DPI
2. Create a custom paper size in the driver.

3. Select custom size as the Paper Size and "Scale to Fit Paper Size" options

4. Confirm the paper features section includes bypass and custom size selection

5. Click Print
The BizHub C220 then prompts me to insert A4 size paper into the bypass some reason. So it's still expecting A4 after all of that. If I go into the paper settings on the printer itself before clicking print, I can setup bypass for a custom size at 13.625 x 10.625. It will then accept the paper when I print. It doesn't complain at all, however the image is screwed and misassigned. It seems like its still trying to scale based on A4. Plus the orientation is wrong too.
So where's the issue, is it print driver or printer?
