try to do it, puting original in the glass not in adf
if the problem hapens again you have a bad DIMM in the controler

is it an ardf or single pass doc feeder? I'm thinking that this model came with single pass, if so, clean the cis of the df

The bar codes look like a memory DIMM issue or maybe a bad CIS.

The light CQ looks like the large white roller springs are stuck. I've gotten similar light CQ after cleaning label gum off of the white roller, but only on one side of the page.

A quick test for the RAM would be to test with different resolutions.

If the problem changes (worsens at higher DPI), that is a near certain indicator of funky memory.

I'm pretty sure it will be the CIS, it looks almost exactly like an issue I had a few years back. If it was a memory problem I would expect the image/text itself to be wonky, but it looks complete.
