The carriage bushings can definitely be the issue. When they are inserted correctly they sit flush. These do not have to be removed, ever, really.
The first image is the carriage bushing inserted correctly. The 2nd image is the bushing inserted incorrectly. The 3rd image is the rear bushing.

I would also check the bushings on the carriage itself. You didn't take those off, did you?

Hmmmmmmmm.......
I'd clean the encoder strip, but, my understanding is the encoder strip only tells the carriage where it's at. I noticed the Cyan printhead says Worn Out and the expiration date is 2014 as a status. Are you able to replace it?Leave a comment:

I'd replace the encoder strip. That's what it looks like to me.
Have you attempted to clean the encoder? I've had some success with alcohol and a cotton pad. Anything stronger than alcohol will remove the silk-screened pattern. =^..^=Leave a comment:
