    Looks liks PH unit(Laser) failure.

Maybe a dusty polygon mirror? =^..^=
If you'd like a serious answer to your request:
    1) demonstrate that you've read the manual
    2) demonstrate that you made some attempt to fix it.
    3) if you're going to ask about jams include the jam code.
    4) if you're going to ask about an error code include the error code.
    5) You are the person onsite. Only you can make observations.

Blackcat honestly I dunno. Dusty poly usually appears as uneven(low) density.
Cleaning PH unit may be a variant.

I did add a code error, it gave a d1 error.
Consulted the manual and followed the procedure to clean set the ADF and still the problem persist.would be checking the polygon mirrow by tomorrow as suggested by some great individuals from the group. Thanks I do appreciate.

Aren't those lines the 'Security patterns' that occurs after improper machine setup/NVRAM issues? This is what I can remember seeing that but I might be wrong... been a while since I worked on KM

The lines of shame are usually cyan or magenta, AFAIK never black. =^..^=
