Also, is it plain paper, glossy, etc?

If fuser isn't worn out, its paper settings issue.

secondary transfer roller is clean, but how to ensure that during printing its contact are intact ?

. Clean the metal contact that touches the shaft near the end and apply conductive grease. Did you do all the proper gradations as I suggested? Could be a bad HVT.. hard to say. Definitely a transfer issue

I was looking at the media types on another device just for comparison. It shows that 256 gsm is considered Heavy 6. Do you have that option when running through the bypass? Heavy 4 only goes up to 180 gsm, for example.

i have Heavy 3 (160-220gsm) and Heavy 4 (220-256gsm) option showing in the control panel
    when feeding through bypass.

there is not heavy 6 option.

Ok, just asking. Have you done a full auto gradation adjustment for heavy 4 paper?

    hello

what you have done is.you told mc to give more bias charge to the 2nd transfer roller.

it may cause a issue with plain.but that you will need to test.
only time when you do this is when 2nd transfer roller is giving a issue with cq.
    you tried to change roller?

Not changed the roller. just cleaned and changed the setting and it works in both plain & thick papers.
