are the paper guides on the tray tight to the paper?
what are the condition of the feed and image parts?

i only see a slight skew in the test pattern, have you initialized and done the both sides adjust for the trapezoid effect?
We know a thing or two because we've seen a thing or two.

hard to tell by your drawing is its skew or trapezoidal.

First you have to measure and adjust double side adjustment, mostly this happen in front side. If not solved, check the first feeding roller in right side of ADU, if the 2 roller pressure was not same, adjust pressure of rollers. Rollers pressure are changes when paper jamming in feeding roller.20191125_211322.jpg
