Have seen the little black plastic pieces that the scanner carriage runs on, break and cause the mirrors to lose focus and cause a wavy image on the page. There's 4 of them, one in each corner. Maybe ones broken and when the carriage moves it's dragging a bit and causing the mirrors to change angle.
If the prints and the copies from the adf are good, then
I would try setting the target with test chart P/N: 302FZ56990 using code u425 ; then run the chart on the glass using u411.
Thank you everyone. I will give those a try.
Thank you for your help. Yes it is the black plastic bushing on the optics itself. It was so small I missed it.
Please do not take offense, but did you clean the optics?
Congratulations on your success. =^..^=
If you'd like a serious answer to your request:
1) demonstrate that you've read the manual
2) demonstrate that you made some attempt to fix it.
3) if you're going to ask about jams include the jam code.
4) if you're going to ask about an error code include the error code.
5) You are the person onsite. Only you can make observations.

It's a real bugger of a one to find. It took me close to 2 hours the first time I saw this happen. In the last 4 years I've had it happen maybe half a dozen times. Still can't work out how they break.
