If you wait about 5 minutes, the first print after that time will also have first 5 cm with background. By copying it is necessary to wait longer. The counter is about 3 millions. I have changed: drum, blade, brush, corona wire, grid, developer, transfer unit. Maybe drum grounding?

Genuine drum? Have you tried in A3 to see if the defect is repeated throughout the sheet? Did you also clean the scanner section thoroughly? That old machine might have a dirty laser too
Another attempt is to wipe the drum with a dry cloth to remove any invisible stains such as halos

My bet is also on the drum. Known problem on this machine series.

When the drum surface starts to deteriorate, it can not reach full charge quickly enough and results in the background on the lead edge of the first page after setting idle. Ricoh drums for this model are rated for 700K. the rating is based on normal usage with letter size paper long edge fed. Longer paper lengths and small jobs increase the average drum rotations per page shorten the drum life. Non genuine Ricoh drums WILL fail sooner and WILL actually cost you more in the long run. Rebuilding the charge corona may give you another week or two.

Did you replace with NEW oem or used

Actual drum is not original ricoh, I will replace it once more with original one. Drum meter reading about 25000 pages. Scanner section cannot be the reason for the fault as the error is on the counter printouts also.

If in fact it is an actual NEW Genuine Ricoh drum for the martini series, then you have a bad HVPS, bad corona or connection between the two. Bad drum ground would produce a solid black band anywhere on the page and some times the entire page and an occasional Service Call code.
