Removed lamp and reset it, still the same.

After looking at the picture you posted, it is not how the lamp is mounted in the holder. It is the scanner wires being out of adjustment. You will need 4 A1849501 Scanner Positioning Pin (2pcs/set). If the rear of lamp carriage and the mirror carriage align to the positioning holes in the frame then the front cable drum has slipped on the shaft. Loosen the locking screw and move the front of one of the carriages until the front locking pins drop into position. Tighten all locking screws before removing the positioning pins.

Also check to see it there is anything obstructing the front end from traveling the entire distance as that may have caused the initial problem.

    I've found the solution.
SENSOR CIRCUIT BOARD D1495102 was the problem. I took time to check scanner section of the machine and it was discovered that the sensor circuit board was out of position, Hell don't how it was out position.

OK nice one - perhaps you got caught in one of those "hidden camra" TV shows

It was really a nightmare trying to figure out the problem. Not knowing it was outside scanner adjustments.
