What have you done to try and troubleshoot so far? Have you tried another ADF to see if the problem remains? If it does, problem with main body, if not, problem with ADF. Simple troubleshooting steps...

Based on the sample, definitely the belt as copyman stated. That is what one of the two calibration stripes look like after the cleaning blade attempts, but fails, to clean it off the belt surface.
Not just shifted, but it looks compressed as well. Not really sure what to say about that. I assume it happen with everything including prints and copies off the glass (not ADF)?
One thing I know that happens when you change that switch is network settings get deleted. There might be some others as well, but I know network does for sure.
