Classic example of unfused toner stuck within the fuser (Most likely on belt separation plate or on web roller):
^ A recent job was run on heavy stock with plain paper setting. Unfused toner heavily contaminates the web roller, subsequently re-contaminating the lower heat roller & belt ongoing.
^ Check Fuser Pressure Springs. MX5141 should have the updated pressure springs, but confirm both are intact and not broken.
^ Check Fuser temperature settings.
^ Confirm correct media/tray settings.
If you remove the web cleaning unit and it is heavily contaminated, you can rotate this forward manually or in sim 43-31.
If this happens frequently, you can increase the speed of the web roller rotation during normal use in sim 43-32. Depending on values used, the web will finish slightly faster than specification, but not by very much and its not a big deal.
Talk to the users. Do they use heavy paper of any kind? If so, its almost a guarantee they've used heavy paper on plain paper setting. Unfused toner contaminates the next sheets exactly like your sample.
To add, also check to make sure fuser settings are set to 23-28lbs as default. It can be found in system settings, device control.
