Check "Trouble Isolation"

The PK-505 is not selectable on the control panel after installation.
Solution:
PROBABLE CAUSES:
1. Incorrect DIP switch settings.
To enable the PK-505, please refer to the attached PK-505 Installation Manual page. To view the PDF, you may need to download a free copy of Adobe Reader®.
Notes:
a. If DIP switch 4-2 is set to 0 (OFF), the Punch Unit may work correctly when printing from the driver, but it is not available on the MFP front panel. Ensure that DIP switch 4-2 is set to 1 (ON).
b. When enabling any DIP switch setting, confirm that the SET button is pressed after each DIP switch change.
2. Failed Punch Driving Board assembly.
Ensure that DIP switches 19-4 and 19-5 are set to 0 (Faulty Part Isolation = NORMAL). If the DIPSW settings are correct then replace the PDB (p/n 15KY H010 00).
Since the punch was working, my guess is a bad control board...My shop guys say that about 25-30% of new setups have bad punch boards out of the box. EMujo
