xerox D95 error code 012-216

xerox D95 error code 012-216. when open copier the stapler in finisher will be move to center and shown code 012-216.
I try to replace all sensor but samthing. has anyone seen anything like this, or may i have NVM to solving this problem.

I know you replaced the sensors, but didn't specify which. Does the end wall home sensor change state in diagnostics? Does the end wall motor energizes? Retension the end wall motor drive belt. Check for obstructions that may cause the end wall from proper operation.
