new installation form

hi, Does anyone have a form that the customer can complete for new mfp installation with details such as network settings, the new ip address of the mfp, administrator username and passwords, settings for scan to folder/email etc. I usually can find this information onsite but sometimes some customers don't have a clue and the IT is unreachable. I think it's a good idea for the customer to fill the form in before the installation with as much information as possible.

Forms won't work with some salesmen, I know one that put Microsoft Office as the OS....LOL
