Not a Canon guy, so if answer is simple please be gentle.......

See attached files. Fax confirmation sheet shows image of fax sent, however all other info is shows zero's. JOB NO., TIME AND PAGES. This information does show up on the Activity Report

How do I get the info to print on Confirmation sheet?

You got me on this one. Maybe it shows the fax and does not need transmit info. Try turning the image off.

Settings on left column
Fax TX- turn off the include image button.

I had this on an IR3045, go to Service Mode and do a CLEAR ALL under FAX.
