After Replacing or Cleaning the Patch Image Sensor, don't forget to execute FUNCTION>MISC-P>PT-LPADJ (this adjusts the initial volume of reflective light for Patch Image Detection).

Check the Sensor Shutter to make sure it moves freely and that the windows for the Front, Center and Rear Sensors are clean.
Check the operation of the Patch Detection Shutter Solenoid SL8 under COPIER>FUNCTION>PARTCHK> SL (8). The Solenoid can be isolated by temporarily removing the Shutter while checking the Sensors.
Check DISPLAY>DENS> PCH-LED1, 2 and 3, each location should have a reading similar to the ones
    below.
    PCH-LED1: 161 (Front Sensor Window)
    PCH-LED2: 211 (Center Sensor Window)
    PCH-LED3: 148 (Rear Sensor Window)

If one or all of the readings for the Front, Center or Rear Sensor are “0”, this is a clear indicator that the Sensor Assembly or its wiring is faulty. Exchanging the position of the Sensors is quick way to see if the faulty Sensor moved and to determine if the Patch Image Reading Sensor should be replaced.
    NOTE:
If one or all the Sensors in the Patch Image Reading Sensor are defective, a new Patch Image Reading Sensor Assembly should be installed.

I have seen defective units sent out-
