IR2200 Fax Kit

In the kit is a main fax board, a speaker and a small board that mounts to the lower left of the fax board. I inherited this kit with a 3 pin connector # J715 yanked right out of the board. Does anyone know what this board is called and if its an available part. Part number on the board is FG3-1607 but nothing comes up for that number in the kit or even google. I could probably just yank one from another board but if its an available part reasonably, I would just buy one to complete this fax kit.

That is your off-hook detect PCB. The correct part number is FG6-5802-000. I just ordered one last week.

Does anyone have the installation directions for the kit? I have one, but I cannot locate online the instructions.
