ir3045 duplex jams on Letter size

Have an IR3045 that jams when duplexing on Letter size paper. Works fine on Letter R. Jam codes are 010A and 010C, when it occasionally completes the duplex on Letter size, the second side of the copy is skewed. We have had a problem with this customer using cheap paper in the past but before we go back over with quality paper to test it, thought I'd see if there were other ideas. We have removed all dampers on door and reg flag.

010A DUPLEX jam can be caused by the tab on the second delivery flapper being damaged or broken, preventing the flapper from operating correctly. The part number for the flapper is FC5-1003-040.

010C DUPLEX JAM/ Make sure the second vertical path roller shaft in the second delivery frame assembly has not dislodged itself.
sdf.JPGAfix back bushing/ check e-clips/ if too much play/or repeated dislodging > u need to replace second delivery frame assembly is FM2-4471-000.

under the second delivery flapper, there are four pieces of mounting tape, remove them and try.
