IR8095 1011 jam

Getting 1011 jam only when stapling in the "booklet finisher D1". Any ideas?

i was press for time when i first posted heres some background on machine.
it has 6 million on it. will run just fine as long as you dont staple. when you staple it will some times run for 100 or so sets some time it will jam every couple sets.does not matter how small or large set are. the shafts and bushings have been replaced. S101 has been replaced. all rollers and S102 have been cleaned. i noticed that sometimes when finisher door was closed it still showed open on screen. i modified switch to close better. when it jams the page from machine is stopped between first two rollers at finisher entrance. maybe some one has had similar issue with a fix.
