It relates to the entrance guide in finisher. Client has a jam in finisher entrance, lowers the tray (forget the FN number lever) then they close it and the mylars get caught. I find if I remove the finisher completely and remove the fuser exit rollers (left side of copier, remove main left metal side cover, then 4 short screws and two connectors. Remove the exit unit and clean the rollers. You will find that over time they get smooth so they need a good clean. Also check under black guide plate to make sure mylars are ok. Basically it's a bit of a bad design, very small tolerance.
