Copystar 6500 error codes 9011 and 9400

Working on a copystar 6500 getting an error code 9011 and 0400, from the ADF. Have replaced ADF board, and sensor. Still have an issue, paper will not go thru all the way. Any ideas?

For the paper not going through, I'd look for a paper clip or such in the paper path.
