Right from the book. Perhaps the Y toner level detection 'window' is dusted with toner and it is trying to add from an empty cartridge. Toner cartridge shutter not opening when installed. Broken toner hopper gear. Defective Y imaging unit. Try running a yellow test pattern and see how the density appears, and think simple first!
