[Misc] KM 4650 envelopes print high & left

I'm starting to see this weekly and haven't found the fix yet. Different printers and users mostly using Word 2003 wanting to print #10 envelopes. The output places the address near the center top of envelope (when using ps driver) or center top and way to the left (when using pcl driver) The return address is basically where it should be. I've tried the latest drivers from KM as well as one or two steps older, no change. Flashed printer to latest version. Changing margins within the application has little or no effect. The envelopes are fed from the top tray, centered by the side guides.
Any thoughts?

Use bypass tray and manually program envelope size.
Had this on a 4035 and it turned out their envelopes were not #10 size, they were 4X9.
