You have a scanner home position error. When you say "other copies are fine" can you copy on the glass in 100% mode. So does it error out when the scanner moves ? If so this is where your problem is.
It is difficult to see, but mirrors do come out of position when transporting.

Right...when i make a copy on the glass in 100% mode it does just fine. No errors. So that makes me believe that the mirrors are not coming out of place while transporting.
