Is the machine actually adding toner, or is the bottle just turning? Can you confirm this? Could also be stripped gears so the toner is not fed into the dev unit and therefore copies stay light even though machine says that it is adding toner....

If the toner was replaced recently, check that there is not the inner cap of the previous bottle, still stuck in the hopper. This would prevent the bottle from being opened and toner coming out.
