Ricoh MP 201SPF Program issue

I have a Ricoh MP 201 SPF and I programmed into "Fax Information" the name of company and the phone number where fax is coming from and when they send faxes, the phone # does not appear on the top of page...everything else does... it is crucial for this company to have their phone # on faxes they send...plz HELP! what am I missing???

Fax header must be turned on and the information programmed into the fax header on the machine. I have included the relevant sections of the user manual.
