    supervisor no password
    Tried admin no password
    Tried admin/admin

    Any chance this machine is displaying an 'Address Book Data Error'? I had one recently where the client couldn't get into WIM and after SP5-846-50 and a restart life was good. If the defaults have been changed for both admin and supervisor you will likely need to replace the NVRAM(s), there is one on the BICU and one on the controller and I'm not sure which one holds the authentication information. You can also try SP8-801-11 to clear the NCS settings, you will loose any network configuration with this SP so print the configuration first User Tools>System>Interface>Print List.

      Have you tried logging into the device itself using "supervisor' and no password? I don't think you can log in to the WIM with supervisor as it is only used to reset admin passwords and cannot change settings. You should be able to log in to the mfp with "supervisor" and reset the admin password. If you can't, then as zed said you will have to change out the nvram.

        actually, you can log in to the web interface as a supervisor. it will give you one choice, "configuration". when you click on that link, it takes you to the screen to reset the password.

        sounds like NVRAm could be in your future.
