Does it still throw this code without an SD card in any slot (I would hope it wouldn't)?*
Try updating/reloading firmware. Maybe the reader has dirty contacts. Or the reader is defective which would require a new controller.

And reading the Goose's guide, possibly the DIMM memory on the controller.

Forgot to say:

- Already Rewrited all Firmwares and Replaced the DIMM , and NO SUCCESS.
- And Without the SD , error still Occurs.
