    You have replaced the normal suspects.

    So try cleaning / testing / replacing the so called drum quenching lamp, which is actually just a thin long PCB of small LEDs.
    And don't forget to empty the reg roller dust collector as it gets pretty full, you don't want any of that transferring back onto the paper as it runs through reg.

        We had this issue too - replacing the PCU (we're using the 1027 as well) > issue returned after a short timespan. Then we've installed the PM-able PCU since ricoh states that it's not recommendable to use the type 1027 since they might not handle the higher process speed. Well - again the issue returned. Finally we ended up with replacing the PCU + HV board + ID sensor and so far we didn't get another call since weeks.

        In case we get another complaint I'd definetly check out the reg-rollers but usually I keep them clean if they look dusty, including the paper dust collector (finally a good accessible construction in this engine). But somehow I doubt that the reg-rollers can cause the issue because this would lead to the question why the spots are gone for a little while after replacing the PCU. On the other hand it might just be a case of coincidence that there were no spots.

            The only time i had a hard time finding spots on this model is when it turned out to be toner on the sponge roller at the trays. I had pm'd it and cleaned the machine but didnt notice toner on the sponge rollers so the paper was feeding out of the tray and picking up spots from there. I didnt catch until i opened the door and feed a bunch of paper out and saw it was junky before even getting to the pcu after i cleaned the rollers it was ok. Otherwise gunk stuck on the fuser entrance and paper exit guide areas where after running enough paper to warm the guides up and get the toner thats on them nice and sticky it marks up the pages when they pass through.

            From the layout of the spots it looks sporadic, it doesnt look like any kind of junk on a roller because that usually results in spots repeating every so many inches across the page depending onthe diameter of the roller, your spots look random on that page like its just smudging somewhere. I guess i would take everything out make sure there isnt any toner anywhere in the machine, check all the guides that the paper follows for any toner melted on. Tough one to guess at for me.

                I have had the same problem and asyou said its not a toner spot i suspect in my case it is paper with a high dust content
                each time you replace the pcu the fault will clear take on eof the pcus with the faualt and empy the dev out then scrape around in the dev and you will see bits of papaer dust remember this pcu recycalls all the wast toner back into the pcu to prove the point exchange a few boxes of paper and see if fault gose away
