I can confirm that you are correct with this issue as we have a site with MPCW2200's and they migrated from 2008 to Server 2012 and the new driver had this exact same issue. I upgraded the PS firmware as you have before flagging it to Ricoh as a driver issue. Here's where the fun started as it flagged to ricoh that i wasn't Wide format trained (i am Fully HP Wide Format trained) they revoked some of our dealer status. Forgive me for responding to your thread as i can't fully answer your question but... I could phone the I.T. guy up at this site but wouldn't want him to say that he still has the issue and could he relog the call kind of thing but i'm pretty sure he did find a solution to the problem by disabling Virtual printer settings.
Effects of the Virtual Printer on device setting and behavior.
If Virtual Printer = Active, some device settings have no effect.
The Virtual Printer's settings will override those settings.

There is nothing that can be done to prevent this other than disabling the Virtual Printer.Virtual Printer=disabled Virtual Printer=enabled If a job is missing the paper type, "Plain/Recycled" is applied automatically. Paper Type setting If a job does not specify the input tray, "Auto Tray Select" is always used. Input Tray setting If a job does not specify the output tray, "Device Default" is always used. Output Tray setting
