Run ACC in User tools. Do Copy and all Print resolutions. If that does not help, replace the ITB.

Have you tried another/fresh stack of paper?

I Second the ACC, I would thoroughly clean all the optics first then when you do an ACC place a few sheets of paper on top of the printout before you scan and you will see better results. Also check the PTR if its past yield i'd change it.

Check the springs that tension the PTR towards the ITB, one of them may be out of position, otherwise it could the the pivot point of the transfer/separation casing.
Have you checked the image on the belt, I'm guessing it's OK, that looks like PTR problem

When I get pale prints on those machines, and ACC fails because of it, I manually adds toner using SP 3015, you sometimes have to run it twice or more, to get it good.

I suspect the cause of the decline, is the developer degrading over time. I almost never have to change dev on the MPC line of machines, and the few that gets pale, I just refresh with SP 3015.

If all fails, replace the developer.

              Hi there!
Agree with Debs, check the PTR-casing. It gets "old" & cracks often.

You can easely check by pushing the duplexunit inwards while printing a test page. if it comes out good (or at least better) it's the ptr pressure.
